Certain "Relevant" Questions Sparks "Error Initializing Form"

I have a survey123 Connect file that get's an error when loaded in the web app, but seems to work correctly at least in the Survey123 Connect test.

The error is: "Error Initializing Form":

TypeError: Cannot set properties of null (setting 'disabled')

What do I know:

1) Through process of elimination, I have determined that the cause of the error is from three rows (20 - canyon_user, 22 - what_areas_resident, 23 - what_areas_non_resident), specifically in their "Relevant" fields.

2) It seems that no matter what I put in the relevant field for those three questions, it still throws the error

3) It doesn't have the error when I delete the contents of the relevant fields

I'm very confused why this might be happening as I have other relevant fields within the survey, and I don't have any special settings on any of those three fields. I have tried publishing from a different computer with an older version of survey123 connect with the same result, I am currently on version 3.21.62.

It seems to be your html formatting in the the list your canyon_areas list.

If you change those two question appearances to compact or blank, it opens on the web app without the type error, but that list takes a lot of real estate, and the compact format is ugly for that so I get not wanting that. Next thing I tried was the html formatting in parks and when that is removed it published and opened with the minimal appearance.

A workaround could be just put "Parks" and then in a hint put all the various types that you have in the html formatted list currently.

@DougBrowning Hmm, I tried that -- creating a blank template and only pasting values. I am getting the same error 😕

I see a bunch of green warning marks in your label column. Did you maybe copy paste from name column? You cannot do that or it will copy all the validation formulas over. You have to right-click paste values instead.
That said I am not sure that would cause what you are seeing.

I also see relevant has no drop down. That makes me think you did other copy pasting.

I would try fixing the sheet. I had some directions on this but all I found was this. https://community.esri.com/t5/arcgis-survey123-questions/data-uploaded-is-not-shown-in-the-data-tab/td-p/1083474 Basically make a new blank template then copy that over to your form and fix each column using Paste Values.
