Common field inputs for multiple surveys

Question

I'm building a survey with approximately 20 fields to fill in by the surveyor, but the data input to the first five fields will not change for a consecutive series of surveys. Is there a way to keep the keep those data inputs for the first five fields so that the surveyor doesn't have to re-enter them every time they do each survey?

Subsequent surveys will have a different set of entries for the first five fields but, again, I'd like it if the surveyor only had to enter them once instead of starting with a blank survey each time.

Once they fill out those first five questions in the first survey, they can save those as "Favorite Answers" and then in a new survey they can "Paste from Favorites".
