Send Error - String or Binary Data would be Truncated

Hello there, 

 

I am getting a "String or binary data would be truncated. The statement has been terminated." for 4 of my fields when I go to submit my survey. I have attached my xlsx and a screenshot of the error code for the fields in question. I read through some discussions and tried bumping the fieldlength up to 1000 and it still fails and says it will be truncated.

 

Please help!

Actually I figured it out, the inspector_checklist needed bumped up. But just an FYI to all, The error message doesn't give the full rap sheet. Look at the diagnostics
