Table with both open and closed questions

Question

Some of my questions are targeting answers that include both open and closed items which should be displayed in a table (see screenshot attached).
Is it possible to construct such a table within the provided excel sheet and if so, how could it be done?
If not, do you have any suggestions on how to display this format best?
