The Schema Preview within Survey123 Connect (v. 2.3.24) is empty for my form. I have tried deleting the form folder and creating a new form utilizing the generic advanced template without success....
Anyone else have this problem?

Adam, those other fields are placeholders, not used during the survey but needed later. The survey is actually gathering data that is ultimately destined for a SQL Server database and those are key fields for that DB. In post-processing (after the survey data are downloaded from the AGOL feature service, and before they are uploaded to the SQL Server DB), the SQL Server DB is queried and those fields filled in with the next unused key values.
This is just one part of a crazy Rube Goldberg-style setup that no sane person would touch, but I inherited it when the designer moved on, and I'm just trying to keep it running until we can redesign from scratch.
Thanks Brandon, your fixes solved all the problems with that form (even one that I didn't mention).
I inherited this survey from its original designer, so I don't know why he didn't use 'hidden' instead of 'calculate' for those other '0' fields, but your suggestion seems like a better approach.
select_one question types don't play well with calculations. I am curious what the "0" value calculations in other calculate fields do though?

I took a look at your XLSForm and see that you have several 'Calculate' fields that have a value of '0' in the calculation column. I'm assuming that this was done in order to prevent the questions from appearing in the survey? If so, I would recommend converting these question types to 'hidden' and removing the calculation value.
After testing with your form, I found that removing the calculation value from your 'select_one WorkUnits' question repaired the Schema Preview. In addition, the question type was missing the '_' in 'select_one'.
Please try to make these changes and let us know your results.

I'm having the same problem, and none of the suggestions in this thread have resolved it. I'm using Survey Connect 2.6.4. The form loads but the schema preview is empty. In addition, the cascading select is not working in the form. I have another form that uses the same cascading select and works fine, yet I can't see anything different between the two forms.
Maybe someone at Esri can take a look? I'm attaching the xls. [Edit: I deleted the attachment after the problem was solved]

Sorry it took so long to reply but I just managed to get the issue fix.
All I did was in Chrome I have to clear the browsing data: browsing history, cookies and other data, cached images and other files.
The Chrome version is Version 63.0.3239.132.

The problem was with the pulldata calculation. After removing it, refreshing my survey, then replacing it in the survey it worked just fine.

Thanks Brandon for taking a look. Since I sent that I have remade the survey and made changes to the CSV file so it would be hard to go back and retrace my steps if it had something to do with the csv file.
When Survey was trying to pulldata or draw from a select_one list it appeared it was referencing incorrect cells, thus making my "relevant" statements on other questions to not function properly. I don't think cell number would matter since its searching for a value, but I did first notice this issue after I had deleted rows in the survey and choices worksheets.
I've seen this with 2 surveys now. I often move rows around within the Survey sheet and make changes to the csv files without issue. Since noticing this problem I no longer delete rows in the Choices worksheet of a survey (I delete values within the rows instead) and I have not experienced the problem since.

After taking a look at your XLSForm, it appears that you are using 'pulldata' to reference an external CSV. As you did not provide either CSV referenced within your form, I removed the calculations referencing them, and refreshed the form. After doing so, the Schema Preview returned. I was then able to re-enter them and refresh, which continued displaying the Schema Preview.
Are you aware of any actions that you implemented in regards to referencing the external content that may have affected the functionality? Can you please try to remove the calculations referencing your external content, refresh the form, and see your results? In addition, please try to re-enter the calculations and let us know your results.
Has anyone had any luck resolving this issue? I've been working on this for the past few days with no progress.
I ran into problems after reordering repeats then when trying to use relevant statements on groups. For both scenarios the questions don't appear in the form preview and the schema preview is unavailable.
I've updated Survey123 Connect to 2.5.44, manually refreshed my survey, and republished my feature service and restarted the survey creation process. I'm able to look at the xml file for the survey, but the schema preview is still empty. I agree with Adam in that the problem could be with reordering or relevant properties (and maybe repeats), but I've done this with earlier surveys and had no issue. I haven't deleted any columns or changed the schema.
Ideas?

When you reply to this message use the "Use advanced editor" option which is located in top right corner of reply window (see below screenshot), and you will see the option to attach file. It sounds like a parameter, calculation or change you have made to the xls template (by deleting columns etc) may have broken the processing Connect does to validate the form. Can you please provide a copy of your xlxs file so we can look into the issue further?
I came across this post looking for answers to the same problem. I'm using survey connect 2.5.44. My survey loads, but Schema Preview window is empty. The excel sheet works ok and no errors in connect when saving, but the survey stops functioning properly (for example relevant statements stop working). The update/refresh icon in survey does not fix it, nor does creating a new survey from the existing template.
It seems it might have something to do with re-ordering rows (cut/insert cut cells) or adding or deleting rows/columns in the survey or choices worksheet (not required/default columns). Hard to pinpoint, I've done all those things before without issues.

What version of Survey123 Connect are you using and on what OS, also what version of Excel? Does the issue occur for all surveys, or just a specific one. Can you share an example of your survey to see if we can replicate?

I'm having the same issue since last week. I'm unable to preview new and created surveys. Any idea if it's going to be fixed.
I'm facing the same issue as James. When I create "New Survey" Converting XLSForm progress bar gets to the end, but never terminates.
I have notice that the xml file in the My Survey Designs / "survey folder" remains at 0 KB... It is probably why the schema previous is empty.
the xml file related to the survey is not created.
Any idea?
The schema is empty as well as the Form Preview.

Just to confirm, you can see the form in Form Preview, but the Schema preview (the listing of fields) is blank, correct?
I actually deleted Connect and reinstalled without any success.

I haven't seen a situation where the schema preview is blank. You might want to try closing & re-opening Survey123 Connect, or 'refresh' the survey design manually.
