We are seeing the antivirus go from single digit to over 50% CPU on our ArcGIS Server system. Does this sound normal?

How often is it going to over 50% usage? If it's just for a short time, then that's pretty normal for an antivirus, typically it either does scans, or random lurches in CPU just because antiviruses are weird. As long as it's not sustained for a very long time, that being over a few minutes at most, then it should be fine.

Yes, antivirus software can cause significant performance constraints on ArcGIS Servers. See here the documentation explaining what needs to be configured in exclusion.

Yes, Antivirus may affect the actual functionality of Esri's ArcGIS Enterprise software. This can range from errors when attempting to join a new machine to a site to performance issues with layers intermittently not loading. Antivirus scan can cause timeouts or even block software processes.
