Is it possible to create a matrix based selection question in Survey 123?

Hello all,

I am looking for the best possible way to create a question in Survey 123 (web/connect) that resembles a matrix having values in rows and columns with specific color visuals.

So, to make it easier to understand, kindly observe the attached screenshot below:

On the left hand side are the two simple questions that we have currently in our form, but we need to have these 2 questions in the matrix format shown at the right hand side.

So, it will be like if a user assumes the probability as moderate and insignificant Impact, the selection would be R3XC1(Low3) and the color has to be dark green and so on.

I doubt if this is achievable and also open to suggestions if its a better idea to keep these 2 questions and have a third text section to just show the output ProbailityXImpact?

Any thoughts on this would be highly appreciated!!

Look though that whole blog again and make sure you're not missing anything.  I'm guessing you haven't setup <path>s within your SVG.

Hello @DuncanC, this is very helpful.

I tried following the workflow and was able to replicate the survey with shared data on the above post.

I would like to create a similar image-map question for a risk matrix, but for some reason I am not able to create one for the below SVG.

It just add the SVG as an image but is not clickable. I might be surely missing something in the survey form configuration.

Do you have any idea how would the choices for this type of risk matrix should be?

Any help on this would be highly appreciated!!
