Table gi_repeat not found? Is it referring to the survey or feature layer?
It looks like you have a repeat in your survey called gi_repeat. Survey123 Connect is trying to find in your Feature Service a table with the same name. You will get the warning above until your Feature Service includes such a table. Alternatively, you can change the name of your Repeat in the XLSFile to match the name of the Feature Layer table in your Feature Service.
The field gi_repeat was given to a begin repeat field type in the xls form.
