I was wondering if I could put a prefix before the GUID. Survey connect has no problem with the calculation formula "GIS_" +uuid() but when sending a survey I get the error code 10500 Database error. Is there proper syntax for this request?

What type of field are you attempting write to? It should be possible via a text question populating a string field sufficiently long enough to hold it (default size of 255 characters is fine).
