Help with select_one_from_file, incorrect column being pulled into survey

Question

I'm using Survey123 connect, I've attached my form and reference .csv.

I've followed the steps in this post but am still receiving the incorrect return on the form (screenshot attached).

I bet it's something simple, I just can't quite get a grasp on it. Thanks for any help!

The lists always display the label column and store the name column.
