Check Boxes with pop-up attributes to fill out

Hi everyone,

I'm trying to enable pop-up attributes to fill out right underneath the checkboxes. Is there a way to do that? I've tried using if statements to display those attributes but they dont display underneath the corresponding checkbox. Instead, they display after the checkbox section. I hope this makes sense.

If possible may we ask for a diagram of your desired end goal? There is a workflow that comes to mind, but want to make sure it aligns with your use case.
