Send Error: A date conversion error has occurred

Question

We are migrating a feature service and a survey from ArcGIS Online to ArcGIS Enterprise 10.5.  I moved the feature service and was able to publish the form to Portal.  When I test submitting data as a user, it fails with "a date conversion error has occurred" error.  Does anyone know what causes this error?

Hi Ahjung,

Are you using a hosted feature service or a feature service tied to an enterprise geodatabase?  Given that this is probably specific to form design and feature service, it might be better to work through Esri Technical Support to address this issue.

I removed all the date type questions, but I am still getting the error.
