I am attempting to use the fileName bind::esri:parameters function to rename images. It is working when the form is submitted through the Survey123 application, but it does not work via the web app. Any help would be greatly appreciated!

Please change `format_date` to `format-date` as suggested in this doc. Then your xlsx file should work in Web app.
