Survey123 wont display in web browser

Hi all,

I am currently making a Survey123 form in Connect, I am quite familiar with the workflow and can usually easily display my survey in the web browser. I'm struggling to get this working, I can display the survey on a mobile device or desktop app completely fine. All the groups (7 groups) show as they should, and drop down isn't greyed out. But when using the survey in a web browser, none of my groups appear and all my drop downs are greyed out. Please see below.

 

 

Any help would be greatly appreciated.
