I'm new to Survey123.

As far as I can tell, there isn't a clean way to put a terms/conditions checkbox and a legal disclaimer at the bottom of a Survey123 form.

Use case: Public government website where the user enters information and photos about historical artifacts to be reviewed and displayed on a public web map.

Could that functionality be added?

Couldn't you just use a note and a required single-choice question? They don't have to end up in the final dataset; you can set the esricolumntype to Null

Add a yes/no required question with your Accept/Consent text as the first question (or add text as a note and then add a yes/no, like @AlfredBaldenweck suggests). Set the relevant field for all subsequent group(s), page(s), question(s), whatever, to only display when the answer is Yes, e.g. ${consent} = Yes. Then no other questions will show.

You could also have a message (note) that's only visible when the answer is No, if you want. I'd also keep the yes/no field in the table for proof of consent.

I have a few forms with this feature already. Esri even has this feature in some of their forms. As others have already given some good solutions, I will just drop a slightly different variant:

Use a select_multiple
Have that list be a single item (e.g., "I Agree")
For that question's label, use HTML tags to include all your required text and/or a link to an external document(s).

Here is a sample screenshot of the various concepts in action. In this particular case, and converse to what I mentioned above (which I would consider the "norm"), I'm actually using an "Agree" and "Disagree" option here. Long storey. But it gets the job done.
