We are using the Serviceability Lookup Solution and getting the following error upon loading the app

The difference is that in the other forum, the error specified which layer was missing. This is not the case for us. All of our layers are sourced correctly, all of the layers that should be in the map are there, and we made sure to configure the "Lookup Layers" tab of the solution.

All of the layers in the map are hosted in ArcGIS Online.
Edit: This has been solved. Our issue was that we did not have the 'Request Tracking' layer within our map. The reason for this was because we are using this layer in an app that is already being used live, so we did not want to test in here to throw our numbers off.
Are you able to share the web map?
Is the layer hosted in ArcGIS Online or a local ArcGIS Server/Enterprise?
