Forms fields with domains not showing choices only blank box

I have Enterprise 11.5. I am working on a simple map that could be used either within Field Maps or Instant App. I have two fields that have domains within for choices. I configured a simple form using the Configure Editing > Forms. In the Forms window, everything looks as it should and Field Maps looks good as well.

My issue is within Instant App or just Editing within MapViewer. When I edit the feature, the popup form just shows a blank box or whatever the existing data shows. It does not show any choice box or drop down. I can only type in data.

I get that forms are generally used within Field Maps.  Is this by design or am I missing something? This happens for both hosted and EGDB referenced data.

Hi @TimWillson

something doesn't seem right. The form in Map Viewer should display the radio buttons, just like you see in the Form builder. I recommend submitting a ticket so we can look into it further.

In troubleshooting some more, I wonder if the exitance of existing data in those domained fields is confusing the form. Originally the data was entered using Survey123, with the coded values of the domain being used. Does Enterprise WebViewer or Forms not recognize coded values, only the label? Maybe best to match both code and label exactly the same as to not confuse it.

After going through some more troubleshooting with support, it seems the issue is with the domains themselves that were originally created within an Enterprise GDB. Even after re-publishing as a hosted feature, it kept whatever issue it had with those domains. If I published a similar dataset and domains from a FGDB and published as a hosted feature, the domains worked as intended in the Forms.
