How to I hide existing feature service fields?

Question

I have an existing feature service that I am using in a survey. I have the feature service all lined up and is working. However, I would like to "hide" the CreationDate, EditDate, GlobalID fields in the survey. I have the fields "hidden" in the ArcGIS Online Item and they are not referenced at all in a survey question.

Any advice on how I can hide them in the survey Inbox?

That was done however it doesn't get reflected in the Survey

Either turn them off in your MXD and republish the service or go to your Web Map and configure pop ups and turn them off there
