How do the entity ids get generated?

Correct. The value is not related to the handle or entity name. It is used for internal lists and processing for ArcGIS for AutoCAD only, arguably not much value other than to distinguish one entity from another in the table.

The ArcGIS for AutoCAD "Entity ID" is an ArcGIS for AutoCAD internally generated read-only value.
