Is it possible to use an existing feature class in an ArcSDE (SQL Server) database for Survey123 results, rather than saving the result to a feature on ArcGIS online?

Hi Robert,
It would be best to move this to the Survey123 for ArcGIS group for more visibility.
In any case, it looks like Survey123 can be used with Portal for ArcGIS. Take a look at this blog post that explains in detail:
