With the added functionality of geospatial PDFs into Field Maps, there is currently only the option of exporting personal markups as .markup (for use in Field Maps only, not AGOL or Pro) or a screenshot (non-georeferenced). The markups only allow "Notes" and "Types" as attributes for the markups.
If exporting only as a screenshot is allowed, perhaps the addition of "label on screenshot by [Field 1]" in this case "Notes" could be added. This way GIS can see labels for the different markups/points that got added and understand what they're supposed to represent.
