In order to use Survey123, you would need at least one user to have the paid Creator license to create and publish the survey itself. In order to fill out the survey, you can sent the web link to anyone and they should be able to fill it out without any requirements.

As @CodyPatterson points out, you need at least a Creator user type to be able to create a survey, and then depending if you are wanting users to fill in the survey using the web app or mobile app, they may need a Mobile Worker user type assigned for the mobile app if the survey is privately shared. However if the survey is shared publicly for anonymous data collection (no ArcGIS user account required) then no user type is required.
