Not sure why I am getting this it was working fine a week or two ago? Trying to run a report on survey 123

Hi David, 

I would make sure that any members of your organization have the correct user type in ArcGIS Online. We went through an update a few weeks ago which included changes to user types. 

It seems like there is an issue with the web map specifically, so I would also check on it outside of Survey123 to make sure nothing has changed recently.
