ArcGIS Online Public Notification Widget - Missing Address Search

Hello,

I'm attempting to use the Public Notification widget on an existing ArcGIS Online Web Application and I noticed the Address Search bar has disappeared from the widget. Any ideas what's causing this? See screenshots for proof this search bar used to exist. I also attached a screenshot of how the widget currently looks (just a blank white space before the point, line, and remove buttons).

Any help would be greatly appreciated. Thanks!
