Multiply questions automatically pick up few answers

I have a problem with multiple selections that never had. I have 5 questions and if click on one the survey picks up automatically 4 answers. Can someone please advise what to do?

Hello, Katherina thanks for all your support. I sorted the issue now was a mistake in the XLS form
