Survey doesn’t submit on iPhone but submits on iPad and desktop

Question

Portal 10.9.1, published from connect, returns these errors when submitting from the iPhone.  iPhone is up to date at software version 15.5.  
tested on android and it sends on android.

Check to see if VPN is disabled on the iPhone.  it should be disabled or turned off
