After creating a survey it says "Submissions are no longer allowed"

I am trying to add a simple survey to collect emails to an ArcGIS open data hub but whenever I create the survey it immediately says "Closed Survey - Submissions are no longer allowed". Am I missing how to initialize it?

What do the settings look like under the Collaborate tab. Is the Survey Status set to "Closed"?
