Crowdsource Reporter Incorrect Date

Hello Everyone,

Using the Crowdsource Reporter template I've created an electric issue submittal form for our constituents to request assistance with varying problems around the city. The feature layer has a Date Submitted field but unfortunately it auto populates to 12/29/1899 with an incorrect time as well (see attached example). I have seen a few pointers online but I really do not understand what they are trying to explain.

Can someone please help me with this issue?
