Multiple radio buttons for select one questions

Question

When publishing a survey using Survey123 Connect Version 3.12.232 the radio buttons on select one questions appear twice (side by side) the righthand bubble is auto selected. The survey functions fine... ie if you click the left button it behaves as expected.

The survey looks fine when previewed in Survey123 Connect.

I've tried the beta version 3.13 and have the same issue.

Thanks for the response. It is a new survey I'm attempting to publish. That is the service we are using (screenshot attached).

Hi @christinesmith602 ,

When was the survey published? If it was published before March 9, you need to republish it to make the choices UI working. Also, could you please confirm that in the Settings - Services - Survey123 REST API URL in Connect, it is using https://survey123.arcgis.com/api ?
