Add data widget

Apologies if this is in the wrong place. I've been runinng a webapp through 10.6.1 portal for a couple of year now. The customer has asked for the add data widget to be added to their webapp which I've done straight out of the box. When I try to import a csv file it fails with the following error:

There was an issue.

Generate Features error: Server tool execution failed : ERROR 999999: Something unexpected caused the tool to fail. Please refer to the error help for potential solutions, or contact Esri Technical Support http://esriurl.com/support. ERROR: unable to connect to the server's managed enterprise database. Failed to execute (Generate Features for Portal). Failed. Failed to execute (Generate Features for Portal). Failed.

A support call has been placed but thought I'd try here too.

@chris_bayleycd , there is a prerequisite of a hosting server to be configured for the Add Data Widget to work.

Thanks I can now upload a .csv however i get a white screen rather than data layering on top of my map. Any ideas?

Unlikely. Can you point me to any setup steps as I've not seen anything about this?
