I cannot connect to survey123.arcgis.com. I have an ArcGIS Online account and try to connect to survey123 but i failed...

1) Can you attempt to log in at https://survey123.arcgis.com (you appear to be logging into http://survey123.arcgis.com ).
2) I see that you have a number of extensions in your browser; we have seen some interference from plugins, like EFF's HTTPS Everywhere, that rewrite URLs automatically. You may want to try disabling any plugin like that.
