Field Maps form with attachments submission fails repeatedly and then photos disappear from the form

Field Maps 26.2.2 Build 2812 - iOS

We have had field teams reporting that the new photo attachment question is causing the form submission to fail, and then the photos will sometimes disappear. I was able to replicate this in office, believe it is related to signal issues, though it seemed to occur even on 5G. The issue is not necessarily that the photos are failing to submit, it is that they disappear from the form after failure.

Workflow that the field team is experiencing -

1. Try to submit a form that has a photo attached to the new "General" question

2. It spins, connection is interrupted, etc. and they get the following error:

"Unable to Submit. 1 attachment failed to upload"

Looking at the troubleshooting logs it says "The network connection was lost."

3. They can choose to either cancel or view the error, which one is chosen does not seem to impact anything.

4. Can try again, but then gets the error:

"Unable to Submit. The operation couldn't be completed. (ArcGIS.ArcGISError error 1.)"

Again the troubleshooting says "The network connection was lost."

4. They try to submit again, it fails again, and then photo attachments disappear from the form.

I don't think that the submission failing is necessarily a problem, working online is subject to occasional network connection issues, but the photos disappearing is causing real headaches for field personnel.

Error: “attachment failed to upload” If connectivity drops during an online submit that includes attachments with Field Maps 26.2.2, the feature edit may save, but the attachment upload can fail, and the attachment remains missing afterward because the upload isn’t reliably retried/queued.

HI @jtmouw_NCDOT , does this error occur when only adding multiple attachments? I noticed in your recording that it mentioned 3 photos failed to submit. Are these new features or are you editing existing features? Also, let me know if this is ArcGIS Online or Enterprise. Just trying to get an understanding of your typical workflow.

Also, have you checked the features online in the Web Map (or reloaded the map in Field Maps) to verify that the attachments are not actually getting sent?

This is in ArcGIS Online hosted data/maps.

From the field, they confirm that it happens with any amount of photos, and I have replicated that as well (see below for 1 photo warning). It can be with new or existing features/related tables - the example attached to the post was a related table, but the attached picture here is a new feature.

They also report that when they back out there are sometimes no photos, sometimes 1 set, and sometimes duplicates - so it sounds like occasionally the photos ARE going through, but is inconsistently. I have replicated that as well - sometimes the photo does go through, sometimes it does not, but the behavior within the form is identical - after the warning shows the photo is no longer visible within the form. Again - in the photo below - it did not go through at all.

They are working in an area that they have covered for the past 4 years where they never had this issue with the previous attachment workflow, so it seems to definitely be pointing at the new attachment question. Anecdotally, they say they don't have the issue in the early morning, but it starts to occur around 9:30, so possibly related to a general uptick in AGOL traffic causing the "The network connection was lost." error, though they are the only team editing their specific dataset. I have replicated it on a number of different maps/datasets, so it is not unique to any one service.

Thank you for those details. I am trying to replicate this scenario and just have one more question. Can you clarify that these were existing maps that were being used before this update? If so, that would mean the General attachment question was automatically added to the Form following the update. Was the question moved at all from the top of the Form?

Yes, these maps pre-existed so the general attachment question was automatically added, and has not been moved.
