I had a similar problem. My survey name had an ampersand "&" in it. I made a new survey without the ampersand and it worked fine. Maybe check that there are no ampersands or special characters in the survey name?
Unfortunately, removing all the non-letter characters (spaces, parentheses) did not fix my problem. The survey still doesn't show up. Thanks for the suggestion, though.
