I found the same behavoir and solution! I can even reproduce the error, if I put the annotation back in the group.
Leader lines disappear at plot or export of map
I found a solution for my particular instance. If the annotation with leaders is grouped in the table of contents with imagery or other layers that have special symbology (eg hatching or transparency) the leaders rasterize when pdf'd. Change the grouping and the text should come out clear and complete.
This is still happening 3 years on. Has there been any response to this yet?
No, it's set to 0. I had one print correctly once, now all attempts leave off half my leaders. The only change I made to the mxd was to add another layer with labels.
You wouldn't happen to have the data frame rotated, such that North is no longer straight up, would you? I have seen similar quirks to what you describe when the data frame rotation was set to anything other than 0.

I am having the same problem as well. Everything looks good in the mxd, but the pdf version is missing random leaders. No setting adjustments have fixed it.
I send these pdf versions out to multiple users and it's quite embarassing to have them looking so unprofessional.
Hi, I realize this has been a while, but I too am now having the same problem. The leader lines are fine within the mxd, but will not export (to anything, not pdf, jpg, etc). Did you ever get this resolved? I'm at an absolute loss. Thanks.
