Add field to Health Screening Dashboard?

Question

I have downloaded and deployed the COVID-19 ESRI Health Screening Solution. Is it possible to add a time stamp to the Temperature Check form? I keep getting a 400 error code when trying to republish the survey with my date field.

I am currently using Survey 123 Connect Version 3.9.120
