Community Maps Address Point Tool is not running correctly, no attribute data

I am trying run the PointAddress tool for community maps to submit new address points but when I run the tool as I have in the past, the resulting point layer has none of the attribute data from the original data set.  The points themselves are moving over but the attribute data is empty. 

I have attached images of the tool before I hit run and the end result.

I am not sure what i am doing wrong in this case.
