Why doesn't my map represent the zoom setting I have set in my survey when I view it on the web? Is there a step that I am missing or is this a functionality that is not supported?

Thanks for pointing this out. The web form you're using is currently in beta (as opposed to the web form available via the Survey123 website). It currently doesn't read the setting from the survey settings. I've added an issue to resolve this.
