Hi Chirag,
What version of Survey123 are you using? Also are you using AGO or Portal (what version)?
Do all iOS devices work ok with same survey, and only Android and Windows do not work? It could be related to a calculation or incorrect data field type or value.
Can you share your xlsx file so we can take a closer look?
