Help on adding feature service definition

Hello,

I am new to Survey123, still testing and learning how to utilize Survey123 Connect. I have created a form for conducting a trailhead inventory. Each time I try to publish the form, however, I receive the following error: "Unable to add feature service definition." I have checked my xls form and name fields and cannot find any issues. Can anyone help resolve this?
