I have tried to follow your advices about how to solve the issue with "Operation Rolled Back" in some repeats option into my form.

Apologies for the delay. I did not see an 'operation rollback' error in submitting with the form you attached. Operation rollback normally occurs when there is too much text for a field or a value is otherwise too large to be stored. It looks like you mostly have integer questions; allowable values for an integer question are between -2^31 (-2,147,483,648) to 2^31-1 (2,147,483,647)
