Renaming a Shapefile in model builder

Question

My model has a final shapefile output. I would like to rename this shapefile from a attribute(s) that have been calculated in the attribute table. for example. I want the shapefile to be named   "clientname_fieldnumber_YYMMDD" I have all those attributes I just need to put them into the final output name.
