Custom forms

Hello all, is there a way to create custom forms that can be used in data collection as in ArcPad Studio (including tabs, check boxes,...etc)?

I need a mobile app that can be used for data collection using custom forms, I couldn't build custom forms with collector app., but I think there should be a way to do that, since it is designed to collect data. What do you think?

I would check out the GeoForms web app. It can only be used for point data currently but the forms look great. It also launches in a browser which I like, no app needs to be downloaded.
