District look up widget error

i was wondering if any one could help me out. I created a school locator app and when I search for designated school I get results but when I expand the information tab of the result I get an error message “ the point associated to the polygon can’t be found. I used the local government model to load data. What am I doing wrong. Please help me. Thanks
