Add image as heading in MapServer page?

Question

I cannot find the question asked previously, but somebody asked how to get an image as a heading in your MapServer page like the National Map page does here: USGSTopoLarge (MapServer)

Can somebody either point me to this GeoNet discussion or explain how this is done in ArcGIS Server?
