Cache mosaic data loss

I have published a mosaic cached image on my Server 10.4. However, I'm experiencing some data loss in between tiles after the creation. Almost as if the the cache didn't complete on certain scales. see below:

Yes, it's showing %100 complete

Did you View cache completion status? Is it showing any of the levels not completed (<100%)?
