Help with Hidden Fields

I apologize for bothering the group but I am stumped.

I created a survey in Connect and have "remnants" of questions that have been deleted. I did save and update the form but the fields still show in a test submission I created

Here is a screen shot 

I have deleted the calculated field for submission ID and I do not see where the "survey" and "TotalCount" fields are coming from

When you publish a form from connect and then republish but delete fields, your feature service is not over-written it just does nothing with the fields omitted in the survey form.  Similar, this is why newly added fields are added to the end of the FS.  You can probably delete those fields from the FS in AGOL without impacting your survey if they are not used and you don't want to see them anymore.  Otherwise you would have to do a fresh republish; save as and publish as a completely new survey without those fields.
