Create Report Survey123 and uploading to One Drive

This flow stopped working on August 28th 2025. I have re-watched the video and still have had zero success. I have refreshed the connection and etc yet it still does not get the file url. There is no where clause either. I have read all the other issue but none of the solutions seem to work or apply to my issue. Am I missing something?

I think one of the most helpful things for troubleshooting is to show the 'outputs' of the actions.

And I think I see the issue. In the report template the planner had added notes to the headers of the sections and they had not been removed.
