Is there any way to style my legend in ArcMap 10.4 as shown in the image.

Sorry to say, I'm not aware of ArcMap direct functionality to style the legend you want. In situations where I need to customize my legend, I will integrate workarounds (e.g. add text, add empty layer names, etc...) to manually create a legend w/ all the required information. Not sure if this helped?
