Portal - New Item Page is Blank

I just noticed that now when I click on Contents > New Item, the pop up page is blank (see attached):

Any ideas?

Did anything happen recently to cause this? Have you checked the Dev Tools to see if any errors show up? Did you try clearing your browser cache?
