Created Survey from Assignment layer can't send

Hello - I created a survey from the existing Assignments layer. No matter how simple the survey is I always get a "Can't send - database error" when submitting surveys. Both Workforce and Survey123 are in our portal enterprise environment. I have full editing permissions so not sure what is going on. Anyone dealt with this issue?
